ORDERING PROCEDURE FOR INDIVIDUAL NON-MEMBERS

An individual non-member subscription to MCP Online allows access for one individual. The cost for 2007 is $355. Individual subscriptions cannot be shared with others. This includes colleagues, students, and lab members. The individual subscription can be used by the purchaser only. If others require access, the institution must purchase an institutional subscription.

Individual non-member subscribers should submit the 2007 nonmember MCP Online subscription order form.

Include your name, address, telephone no., fax no., and email address with the order, along with credit card number and expiration date if this is your payment method. After your payment is received, you will receive an email notification with instructions for obtaining access to MCP Online; the letter will direct you to an on-line activation form. (You must include your current email address with your subscription payment.) The on-line activation form will prompt you to provide the following information necessary to activate your subscription to MCP Online:

• Create a user name and password that you can easily remember
• Your name, phone number, and email address
• Your customer number (included with the activation letter)
